Consumer Health Data Privacy Policy
Effective November 5, 2026 · Last updated November 5, 2026
This policy explains how Apollo Health Clinic collects, uses and shares consumer health data through this website, the online booking process and our email updates. It is written to meet Washington’s My Health My Data Act (chapter 19.373 RCW). We apply the same practices and rights to every visitor, wherever you live.
This policy does not cover health information you share as a patient after you begin care. That information is handled under our patient privacy practices and the health care privacy laws that apply to it.
1. What “consumer health data” means here
Consumer health data is personal information linked or reasonably linkable to you that identifies your past, present or future physical or mental health status. That includes information showing that you are seeking health care services.
2. Consumer health data we collect and why
| What we collect | Where it comes from | Why we collect it |
|---|---|---|
| Your name, email and the fact that you booked a free discovery call, plus the email addresses of any guests you add and anything you choose to write in answer to “Please share anything that will help prepare for our meeting.” | You, through our booking page (Calendly) | To schedule and hold your discovery call and to contact you about it |
| Your name, email and anything you choose to write in a message | You, through our contact form | To reply to you |
| Your name and email, and the free resource you asked for | You, through an email signup form | To send the resource and the email updates you asked for |
We collect this information only because it is necessary to provide the service you asked for. We do not use it for any other purpose without first asking for your consent.
What we don’t collect:
- Quiz answers. Our quizzes run entirely in your browser. Your answers and score are never sent to us or saved. We only count that a quiz was finished.
- Location. We don’t collect your precise location. We don’t use geofencing.
- Advertising data. We don’t use advertising cookies, tracking pixels or targeted advertising.
- Browsing history. Our website analytics (Cloudflare Web Analytics) count page views without cookies and aren’t linked to your name, email or identity.
Please don’t send detailed medical information through our website forms. Share health details only during a scheduled visit or through the patient portal you receive as a patient.
3. Sharing consumer health data
We do not sell consumer health data.
We share consumer health data only with service providers that process it on our behalf to provide the service you asked for. They may use it only for that purpose, under contract. We have no affiliates.
4. Categories of service providers
| Category | Provider | What it receives |
|---|---|---|
| Scheduling | Calendly | Your booking details and booking answers |
| Video calls | Zoom | Your name and email for the discovery call link |
| Email updates | MailerLite | Your name, email and signup details |
| Website hosting, security and spam protection | Cloudflare (including Turnstile) | Technical data needed to deliver the site and block automated spam |
| Clinic email | Resend (delivers form messages) and Google Workspace (the clinic’s mailbox) | Contact form messages, delivered as email |
We may also disclose information when the law requires it.
5. Your rights
You have the right to:
- Confirm and access. Confirm whether we collect, share or sell your consumer health data, and get a copy of it.
- See who received it. Get a list of every third party and affiliate we’ve shared or sold it to, and how to contact them.
- Withdraw consent. Withdraw any consent you gave us to collect or share your consumer health data.
- Delete. Have your consumer health data deleted, including by the service providers that hold it for us. Copies in backups are deleted within six months.
We won’t treat you differently for using these rights.
6. How to make a request
Email [email protected] with the subject line “Health data request,” or send a message through our contact page. Tell us which right you want to use. We may ask for information to confirm your identity, and we use it only for that purpose. You don’t need an account.
We respond within 45 days. If we need more time, we’ll tell you within those 45 days, and we may take up to 45 more days when reasonably necessary.
To unsubscribe from email updates at any time, use the unsubscribe link in any email.
7. Appeals
If we deny your request, you can appeal by emailing [email protected] with the subject line “Health data appeal.” We’ll answer within 45 days and explain our decision in writing. If we deny your appeal, you can contact the Washington State Attorney General: www.atg.wa.gov/file-complaint.
8. How we protect and keep it
Only people who need consumer health data to do their work can access it. We keep booking and contact information only as long as we need it to schedule and follow up with you, or as the law requires. You can ask us to delete it at any time.
9. Changes
If we change this policy, we’ll update the date at the top. Before we collect new categories of consumer health data or use it for new purposes, we’ll update this policy and ask for your consent.
